Provided your permanent address falls within our practice boundary, you have the right to apply to join our practice. We are unable to keep patients on our list who move out of this practice area.
Catchment Area
We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.

- Please make sure you have completed all the relevant details on the form. If you are from abroad you MUST complete that section on the form.
- Complete a separate form for each person.
Registration Forms
Due to the volume of registrations we receive your registration may take a while to be processed by us. Please bear with us and do not contact the surgery regarding your registration request.
When we have accepted you onto our list of patients, your details will be sent to the Local Health Authority and your medical records will be forwarded to us from your previous GP.
Therefore without your previous GP details or date of entry to the UK, we cannot process your registration and it will be rejected.
Covid Passports
*Please note that you can not access covid passports as a proxy user for Children under 16 at the surgery, you will need to contact Children under 18 and the NHS COVID Pass – NHS (www.nhs.uk) for more information. *
